General requirements for a physical therapist license?

• A complete and legible application form with a recent, unmounted, full-face photograph.
• Proof of Lawful Presence in the United States.
• Payment of the fees set by the Board.
• An official transcript giving evidence of graduation from an nationally accredited program approved by the Board.
• Passing scores on the National Physical Therapy Examination (NPTE).
• Verification of license/certificate in good standing from all jurisdictions in which the applicant has been licensed or certified.
• A successfully completed state jurisprudence examination with a passing score of at least 75 percent.
• Two professional letters of recommendation attesting to the applicant’s good moral character and professional abilities.
• License and certificate holders from other jurisdictions applying for a license or certificate in Wyoming must provide evidence of completion of 15 hours of continuing competence credits within the last year.
•In order to obtain state and national criminal history record information, submit legible fingerprint cards.
• Evidence of passing a board-approved English proficiency examination if the applicant’s native language is not English
