Welcome!

Thank you for taking the time to complete this intake form. This is the first step in helping me get to know you, your dog, and the challenges you’re currently experiencing.

Please provide as much detail as you can. The more information you share, the better I can understand your goals, identify factors that may be contributing to your dog’s behavior, and recommend the training program that best fits your needs.

You will also notice a few questions about your own experience and knowledge. These are not a test—there are no right or wrong answers. They simply help me understand what information you already know, where I may need to provide more education or support, and how I can tailor my coaching to best meet your needs.

This is just the beginning of the process, so don’t worry if you can’t think of everything. We’ll discuss your responses during your phone consultation, and I’ll ask any additional follow-up questions as needed.

We consider your dog’s history, health, environment, daily routine, emotional well-being, and your goals as their owner. By understanding both ends of the leash, we can create a training plan that supports your dog as a whole and gives you the knowledge and skills to help them become the best version of themselves.

Not crate trained

Very comfortable - will rest, nap & relax, able to eat even if door is closed.

Tolerates it for short periods, but may whine or paw at it.

Shows stress ( panting, barking, not eating, not really relaxed, escape attempts.

Cannot tolerate the crate at all - panics or becomes very distressed, might injure themselves
