• Be a graduate of a physical therapist or physical therapist assistant educational program which is accredited
• An official transcript which contains the school seal from an accredited school. The transcript must contain information that the degree received was in physical therapy.
• Foreign educated applicants must demonstrate proficiency in written and spoken English.
• One signed original professional reference letter, dated within six months of the filing of the application, addressing, good ethical and competent professional conduct.
