Appointment Request Form
• Please be as specific as possible when filling out this form.
• Please allow us 1-3 days to get back to you. (We normally get back to you the same day.) At this point, we’ll discuss your design and give you a cost estimate.
• Once we know how much time we need for your tattoo, we’ll send you a link to our calendar to book the actual appointment for your tattoo. That appointment requires a minimum deposit of $80, which you will pay online at the time of booking. Cash is accepted if booking in person at the shop.
• We accept Cash, Credit, or Zelle for tattoo payment.
• If you wish to pay with a credit card, you will be responsible for a 3% processing fee.
• You will also be asked to sign a contract that states you have read through and agree to all shop policies. PLEASE read it so there are no surprises!
